Establish an Individual Retirement Account. Review the appropriate Custodial Agreement below prior to submitting the IRA Account Application. - IRA Transfer/Rollover Request Form
Transfer or Rollover a Retirement Account at another financial institution to your U.S. Global Investors IRA account. - IRA Distribution Form
Request a distribution (redemption) from an IRA Account. - Designation of Beneficiary Form
Change the Beneficiary designation for your IRA account. - IRA Roth Conversion Form
Convert a Traditional, SEP or SIMPLE IRA account to a Roth IRA account. A completed IRA Account Application is also required if you do not have an existing Roth IRA account with U.S. Global Investors. - Request to Recharacterize an IRA Contribution Form
Move a Traditional IRA contribution to a Roth IRA or vice versa. An IRA Account Application is also required if the receiving account does not already exist. - IRA / ESA Correction of Excess Form
Correct a contribution made to an IRA or Coverdell ESA account. - IRA Account Application Addendum Form

Establish salary deferred contributions for SIMPLE IRA’s. Employer use only; do not submit to U.S. Global Investors. - SIMPLE IRA Notification to Employees
Distributed annually by employer to all employees eligible to participate in the employer’s SIMPLE IRA plan. - SIMPLE IRA Plan Document
Review by employer prior to establishing a SIMPLE IRA plan. Do not submit to U.S. Global Investors. - IRA and ESA Investment Slips
